In partnership with our clients, we develop and use mobile technology for nurse care management programs.   Department Overview: The clinical team at Wellframe works closely with all other Wellframe departments to create clinical tools, programs, and processes that help patients gain access to the knowledge and clinical expertise they need to manage their health needs at any given moment. Our team is made up of clinicians, communications, patient advocacy, and public health experts who enjoy working with colleagues from diverse backgrounds to solve tough health problems.   Position Overview:  We are currently seeking applicants with a background in patient advocacy, case management, health coaching, health promotion, and/or social work who are interested in joining our team of health advocates. The health advocate will leverage his or her patient communication and health coaching skills to support patients with chronic disease through the use of the Wellframe mobile platform. The role of the Health Advocate is essential to our efforts both to increase engagement with the Wellframe app and to improve health outcomes among our users.   The Health Advocate will work as a part of a multidisciplinary team. Utilizing established evidence-based protocols, the advocate will provide non-clinical health coaching services and support to patients who are enrolled in Wellframe. Expertise in health promotion and/or health coaching is required to facilitate relationship-building with the patient, and to offer an appropriate level of non-clinical support. This role does not involve assuming responsibility for medical care of the patient, or addressing any clinical needs. Clinical support and supervision will be provided by the Wellframe team of clinicians.   Requirements:  -Fluency in both Spanish and English is required -Bachelor’s degree in social work, health education, health promotion, or related field -Strong written and oral communication skills -Experience providing health education counseling and coaching for a variety of chronic and acute health conditions -Familiarity with typical patient post-op recovery experience and monitoring over ensuing weeks is a plus -Prior experience with health technology products is a plus Wellframe, Inc. is an equal opportunity employer. Departmental Overview:  The Client Services team leads the transformation of clinical services with our clients through our mobile-enabled technology, analytics, and innovative approaches to workflow redesign - either directly with payers and providers or through medical device, behavioral health, and other health care partners. This team is responsible for successfully implementing the Wellframe solution with new clients, managing ongoing client relationships, and identifying opportunities for account growth and expansion. Job Overview: The Client Operations Associate is the first line of response for inbound communications via our helpdesk, providing assistance and support to our clients’ clinical staff using the Wellframe dashboard and patient end-users using the Wellframe mobile app. In addition to responsive support communications and becoming an expert in our products and client implementations, the Client Operations Associate will be critical in building and maintaining the Wellframe resources and tools that our clients’ clinical staff need to succeed, such as our online help resource center and training materials.
