Hospice Field Staff RN/Case Manager Position Summary
The Staff Registered Nurse is responsible for assessing the patient/family/caregiver needs, planning for the care, implementing the Plan of Care, and evaluating the effectiveness of the care provided. As a member of the IDT, the Staff Registered Nurse/Case Manager oversees the direction and coordination of a patient’s care throughout their time in hospice.
Pay : $39.21 Hourly. Full-Time Non-Exempt Position. 40 hours weekly/80 hours bi-weekly.
Schedule : Monday- Friday 8:30 AM- 5:00 PM with an additional rotational on-call schedule that includes some evening, weekends, and holidays. As we have on-call nurses, on-call would be limited.
Location : Will be based out of our Belleville Patient Services Office. Would primarily visit patients and travel within the Belleville office’s service area southern service territory, which include the following counties: St. Clair, Monroe, Randolph, and Washington
We visit our patients wherever they may be located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, a valid driver's license, current automobile insurance, and reliable transportation are requirements of the position. The company reimburses .70 cents per mile for driving personal vehicles.

Education
Graduate of State-approved school of nursing
Associate Degree in Nursing or Diploma in Nursing required.
Experience
One (1) year experience in hospice preferred.
Other Qualifications
Current Illinois nursing license.

Prolonged periods of sitting at times. Exerts up to 100 lbs. of force occasionally, and/or up to 50 lbs. frequently and/or 20 lbs. constantly. Requires physical flexibility of bending at knee and squat-bend. The Staff Registered Nurse manages stress appropriately, makes decisions under pressure, handles multiple priorities, works with an Interdisciplinary Team and manages anger/fear/hostility/violence of other appropriately. This position requires clarity of vision >20 inches and <20 feet, hears alarms, telephone/tape recorder.
Working Conditions: The Staff Registered Nurse/Case Manager spends approximately 20% of his/her time in an air-conditioned environment with varying exposure to excessive humidity and noise. The remaining 80% of his/her time is spent in an automobile or patient home, which may or may not have heat/air conditioning. The position has potential for exposure to malodorous odors, infectious body fluids from patients, animal pet hair, allergens in the home environment, some exposure to noxious smells from cleansing agents, and exposure to communicable diseases and traffic.
Essential Functions of the Field Staff RN/Case Manager Position :
Maintains a caseload of patients. Oversees the direction and coordination of the patient’s care and the care provided for their caregiver/family.
Provides direct patient care that includes all aspects of the nursing process, including participation of on-call after hours care. Work on-call in absence of on-call nurse and when need arises.
Demonstrates required registered nurse clinical skills. Gives a practical exhibition and explanation at the required annual skills day.
On each visit completes a physical assessment, medication review, and a symptom and pain evaluation. Accurately and timely enters medication orders. Communicates findings to the hospice physician and obtains orders to manage care. Timely ordering of medications from pharmacy.
Communicates pertinent information to all disciplines by communicator, telephone, encrypted text messaging system, and at the IDT meeting.
Reviews and updates the Plan of Care (POC) on each visit.
Responsible for initiating, supervising and updating the Hospice Aide (HA) plan of care (POC) on a timely basis and supervising visits at least every 14 days per Medicare regulation.
Knowledgeable in patterns of disease progression at end of life and provides effective ongoing patient/family/caregiver education.
Knowledgeable in principles of end of life pain and symptom management. Considers patient/family/caregiver stated values and goals when considering treatment options ensuring their patients are as pain-free and symptom-free as possible.
Identifies the physical, psychosocial, emotional, and spiritual needs related to the terminal illness. Collaborates with IDT to meet those needs.
Documents the elements of each assessment in a systematic and timely manner according to the Policies and Standards of Performance.
Oversees the activity of the Licensed Practical Nurse (LPN) as they provide patient care.
Participate in on-call after-hour care. In the on-call role, the Staff Registered Nurse/Case Manager triages calls and demonstrates critical thinking skills as they prioritize and address each situation.
In an emergency/crisis event, the Staff Registered Nurse/Case Manager demonstrates the ability to take control of a situation, understands and prioritizes care needs, and executes a plan quickly.
Provides emotional, spiritual, and psychosocial support to patients/family/caregivers. Establishes a therapeutic relationship with the patient/family/caregiver.
At the IDT meeting, at least every 15 days, the Staff Registered Nurse/Case Manager shares new information concerning the patient’s disease progression, discuss how to further improve the patient’s quality of life and comfort, and ensures that the care plan is consistent with the patient’s end of life core wishes.
Demonstrates the ability to work cooperatively with others in a team environment to ensure deadlines, goals, and objectives are met.
Demonstrates the ability to embrace change, manage stress and maintain a positive attitude.
Builds relationships and collaborates effectively with internal and external customers while cultivating referrals and referral sources. Identifies and analyzes problems in the referral/admission process and offers solutions for better customer service and satisfaction. Participates in QAPI activities. Knowledgeable about performance improvement measures and works to maintain compliance.
Organized and works efficiently with minimal supervision. Is a self-starter who demonstrates flexibility, motivation, initiative, and commitment. Ensures required assigned education is finished in the specified time.
Identifies and contributes to the development of new ideas and approaches to improve the work process.

Position Overview
The Division of Behavioral Health and Recovery is pleased to announce an opening for a Clinical Nurse Manager (CNM) for the Forensic Units AFC, evening shift at Alton Mental Health Center located in Alton, Illinois. The Alton Mental Health Center is seeking to hire an energetic and detail-oriented licensed Registered Nurse to direct and oversee para-professional and non-professional nursing personnel, assuring that general health and nursing care needs are met for our patients. Alton Mental Health Center strives to provide equitable access to social/human services, supports, programs and resources to enhance the lives of all who we serve.
Essential Functions
Serves as Clinical Nurse Manager for the Forensic Units AFC for the evening shift.
Serves as full-line supervisor.
Provides recommendations to the DON in the development of policies and procedures.
Evaluates patient services by conducting ongoing audits (e.g. performance improvement) of services provided in adherence to established goals and standards.
Provides professional nursing services to patients including new admissions, administering medications, giving shots, taking medical histories, etc.
Develops a plan to implement alternatives to restraint/seclusion which outlines the roles and responsibilities of program staff.
Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
Requires licensure as a Registered Nurse by the State of Illinois.
Requires knowledge, skill and mental development equivalent to the completion of four (4) years of college with coursework in nursing.
Requires prior experience equivalent to three (3) years of progressively responsible professional nursing experience in a public or business organization.
Preferred Qualifications
Three (3) years of professional experience supervising professional nursing staff, recommending and implementing disciplinary actions following collective bargaining agreements.
Three (3) years of professional experience meeting deadlines with strong attention to detail and the ability to utilize critical thinking skills while multi-tasking multiple projects.
Three (3) years of professional nursing experience working with individuals with mental illness.
Three (3) years of professional experience recommending or preparing treatment plans for patients.
Three (3) years of professional experience communicating with internal and external stakeholders in oral or written form relative to the care and treatment of patients.
Three (3) years of professional experience recommending and implementing nursing policies and procedures following evidence-based practice and nursing standards.

Serves as full-line supervisor.

Minimum Qualifications

Job Summary:
Reporting directly to an Assistant Director of Advising, as a Bilingual Advisor, you will provide proactive, student-centered advising that supports student success, retention, and completion from a student’s first point of contact through graduation. As part of Front Range Community College’s commitment to proactive, relationship-based coaching and advising, this position will play a vital role in fostering belonging, confidence, and clarity for every student.
Bilingual Advisors will be assigned to one of two complementary tracks:
New Student Advising: focusing on onboarding and first-semester success.
Student Success Advising: focusing on ongoing planning, goal-setting, persistence, and completion.
Both tracks are grounded in an equity-minded, holistic coaching approach that emphasizes proactive outreach, individualized support, and collaboration with faculty and campus partners to help students achieve their educational and career goals.

The Division of Developmental Disabilities is seeking to hire a Center Director for the Murray Developmental Center located in Centralia, Illinois. Essential Functions
Serves as Center Director for Murray Developmental Center. Preferred Qualifications (in priority order)
Five (5) years of professional experience organizing, administering and evaluating ongoing services in a multi-disciplinary operation.
Five (5) years of professional experience interacting with and communicating in oral and written form with internal and external stakeholders including the use of computers.
Five (5) years of professional supervisory experience assigning work, providing guidance to subordinates, recommending counseling and/or discipline following collective bargaining agreements, training staff, preparing and signing performance evaluations and approving time off.
Five (5) years of professional experience developing and interpreting policies and procedures for a public or private organization.
Five (5) years of professional experience working with Federal and State standards and regulations relative to residential care and treatment programs.
Five (5) years of professional experience developing strategic plans, long and short-term goals for a residential treatment program.
Five (5) years of professional experience interpreting, recommending and designing and implementing staffing plans for a public or private organization.
Master’s degree from an accredited college or university with a specialization in clinical social work, psychology, medicine, hospital administration or a closely related field.

The incumbent will be responsible for all clinical psycho-social and rehabilitation programs and active treatment, development, and rehabilitation of persons with a mental illness. The Division of Developmental Disabilities is seeking to hire a Clinical Director for the Fox Developmental Center located in Dwight, Illinois. The incumbent will be responsible for all clinical psycho-social and habilitative programs and active treatment, development and rehabilitation of persons with a developmental disability. Scores and interprets both objective and projective psychological evaluations for assigned caseload.
Completes psychological assessment upon an individual’s admission to the Shapiro Center and every five (5) years thereafter unless more frequent assessments are clinically indicated.

Position Overview
The Division of Developmental Disabilities is seeking to hire a Psychologist 2 for the Shapiro Center located in Kankakee, Illinois to provide intensive psychotherapy and in-depth counseling to individuals living at the Shapiro Center. Develops and executes treatment plans. Documents treatment. Serves as a member of the Interdisciplinary Treatment Team. Serves as a resource to less experienced Psychologists.
Essential Functions
Provides intensive psychotherapy and in-depth counseling to individuals living at Shapiro Center.
Serves as a member of the Interdisciplinary Teams by conveying individual progress information to the Team for assigned individuals.
Scores and interprets both objective and projective psychological evaluations for assigned caseload.
Completes psychological assessment upon an individual’s admission to the Shapiro Center and every five (5) years thereafter unless more frequent assessments are clinically indicated.
Completes required reports, schedules, forms and correspondence in a timely manner and by the established due dates.
Provides training and instruction to direct care and professional staff with emphasis on behavioral modification and skill development.

Minimum Qualifications
Requires knowledge, skill and mental development equivalent to a doctoral degree in psychology from a recognized college or university supplemented by one (1) year of professional experience in psychology. For candidates not possessing these qualifications, a master’s degree in psychology from a recognized college or university is a minimum requirement.
If the minimum educational requirement is met, four (4) years’ qualifying professional psychology experience may be substituted for the doctoral degree in psychology and one (1) years’ experience.

Position Overview
The Division of Developmental Disabilities is seeking to hire a Social Worker III for the Mabley Developmental Center located in Dixon, Illinois to serve as the transition coordinator for the center and oversee admission, discharge and transitional services. Chairs committees. Performs complex social work services for individuals on a facility-wide basis. Serves as designated lead worker to lower-level staff. Serves as the professional social work resource member of the Interdisciplinary (ID) Teams. Co-chairs the Center’s Professional Staff Team. Serves as the primary team liaison with other disciplines, guardians, residential unit staff, family and community agencies in individual admission and community placement processes.

Position Overview
The Division of Mental Health is seeking to hire a Physician Specialist Option A for the Madden Mental Health Center located in Hines, Illinois to serve as a non-administrative Illinois licensed Physician, practices Family Practice medicine and serves as a primary physician and clinically directs a caseload on an inpatient pavilion. Verifies treatment of patients meets standards of care. Evaluates assigned cases at Intake; signs certificates for involuntary admission and testifies in court. Makes daily rounds. Provides back-up coverage. Serves as an active member of the medical committees.
Essential Functions
As a non-administrative Illinois licensed Physician, practices Family Practice medicine and serves as a primary physician and clinically directs a caseload on an inpatient pavilion.
Makes daily rounds evaluating patients’ progress and treatment.
Provides back-up coverage of other pavilions and assessments at Intake.
Serves as an active member of the medical committees, pavilion and program meetings, case evaluation, audit and utilization review committee to provide expertise in the areas of treatment, development, etc.
Attends and participates in all mandatory training.
Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
Requires a valid State of Illinois Physician and Surgeon License.
Requires completion of an approved residency in the position related medical specialty of Family Practice medicine.
Preferred Qualifications
One (1) year of professional experience evaluating and providing quality medical care to adult patients.
One (1) year of professional experience evaluating, diagnosing and treating adult individuals in accordance with best practices.
One (1) year of professional experience coordinating medical services with other professional disciplines and supportive personnel.
One (1) year of professional experience communicating effectively both verbally and in writing.
One (1) year of professional experience preparing comprehensive and specialized medical records and reports.
One (1) year of professional experience treating adults with mental illness.
Conditions of Employment
Requires the possession of a valid Illinois Controlled Substance License.
Requires the possession of a valid United States Drug Enforcement Administration Certificate of Registration.
Requires the ability to meet the requirements of and be appointed to the medical staff association of the Madden Mental Health Center within 30 days of employment.
Requires the ability to travel in the performance of job duties.
Requires the ability to walk and stand for a significant period of time, including stooping, bending, lifting or exerting up to 20 pounds of force occasionally.
Requires the ability to participate in patient management interventions that may include participating in restraint interventions.
Requires the ability to maintain current Basic Life Support Certification.
Requires the ability to testify in court.
Requires the ability to serve as Medical Officer of the Day (MOD) after business hours, weekends and holidays in case of emergency.
Requires ability to pass the IDHS background check.
Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
