The Division of Behavioral Health and Recovery is seeking to hire a Physician Specialist, Option D, for the Madden Mental Health Center located in Hines, Illinois to serve as a staff psychiatrist. Clinical directs a caseload on an assigned inpatient pavilion on an assigned shift. Ensures psychiatric treatment of patients meets standards of care. Evaluates assigned cases at intake. Provides clinical directions to staff involved in direct care. Provides back-up coverage of other pavilions. One (1) year of professional experience evaluating the quality of specialized medical care and treatment for patients with a mental illness.
One (1) year of professional experience coordinating medical services with other professional disciplines and supportive personnel.
One (1) year of professional experience conducting comprehensive psychiatric evaluations of patients.
One (1) year of professional experience identifying the most appropriate positive treatment option and carrying out that treatment in accordance with best practices.
One (1) year of professional experience communicating effectively both orally and in writing.

Position Overview
The Division of Mental Health is seeking to hire a Physician Specialist Option B for the Madden Mental Health Center located in Chicago, Illinois to serve as a primary care physician. Clinically directs a caseload on an inpatient pavilion. Verifies treatment of individuals meets standards of care. Evaluates assigned cases at Intake.
Essential Functions
Serves as primary care physician serving patients living on an assigned pavilion.
Provides clinical direction to staff involved in direct care.
Provides back-up coverage of other pavilions and assessments at Intake.
Serves as a member of the medical committees, pavilion and program meetings, case evaluation, audit and utilization review committee to provide expertise in the areas of treatment, development, etc.
Attends and participates in all mandatory training. Travels in the performance of job duties.
Performs other duties as assigned or required which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
Requires a valid State of Illinois Physician and Surgeon License.
Requires American Board Certification in the job-related medical specialty of Family Practice.
Preferred Qualifications
One (1) year of professional experience evaluating and providing quality medical care to adult patients.
One (1) year of professional experience evaluating, diagnosing and treating adult patients in accordance with best practices.
One (1) year of professional experience coordinating medical services with other professional disciplines and supportive personnel.

Transitions of Care Specialist is responsible for optimizing transitions of care processes for APLA Health patients transitioning from an emergency department (ED) or hospital back to the clinic.

Develop processes to obtain the most timely and complete data possible regarding APLA Health’s patients receiving care in emergency departments and hospitals. Sources of data may include health information exchanges, such as LANES, health system portals, Managed Care Plan/Independent Physician Associations portals (MedPoint Manangement or Cozeva) or direct communication from IPAs (patient discharge PDC reports).
Track patients who have been admitted to a hospital or visited an emergency department, and maintain accurate records on linking patients back to their primary care team at APLA Health. Communication with the responsible APLA Health care team will ensure the care team is aware of the patient and the TOC Specialist will continue to monitor the process until each patient has been triaged. The TOC Specialist will obtain records of the ED or hospital visit and upload them to the patients’ charts as soon as possible. They will schedule follow up appointments for the transitioning patients according to the instructions of the care teams.
Work in conjunction with the enrollment team for eligibility checks for newly assigned patients, particularly those identified through ED or ER notifications but not yet seen, and ensure they can be promptly be followed up by a PCP at the CHC.
May coordinate resources to improve patient ability to adhere to appointments, including assistance with transportation.
May screen patients for social needs and help link patients to APLA Health or community resources to address identified needs.
May work with care teams at all APLA Health medical clinics to optimize local community resource guides to be used to help link transitioning patients to the most appropriate local community resources.
Monitor and report IPA data related to ED/Hospital discharges and prepartum/postpartum care to ensure accuracy of data and to ensure that patients are receiving timely follow up to meet key quality measure goals.

Position Overview
The Division of Developmental Disabilities is seeking to hire a Physician Specialist Option B for the Kiley Developmental Center located in Waukegan, Illinois to perform general medical examinations, provides diagnostic and treatment services. Provides facility wide comprehensive medical care, treatment and diagnosis of disorders relating to Family Medicine specialty to medical staff. Verifies treatment of individuals meets standards of care. Evaluates assigned cases at Intake. The Division of Developmental Disabilities is seeking to hire a Physician Specialist, Option B for the Shapiro Center located in Kankakee, Illinois to perform general medical examinations, providing diagnostic and treatment services. Provides facility wide comprehensive medical care, treatment and diagnosis of disorders relating to Family Practice to medical staff. Verifies treatment of individuals meets standards of care. Position Overview
The Division of Behavioral Health and Recovery is seeking to hire a Physician Specialist, Option B for the Madden Mental Health Center located in Chicago, Illinois to serve as a primary care physician. Clinically directs a caseload on an inpatient pavilion. Verifies treatment of individuals meets standards of care. Evaluates assigned cases at Intake.
Essential Functions
Serves as primary care physician serving patients living on an assigned pavilion.
Provides clinical direction to staff involved in direct care.
Provides back-up coverage of other pavilions and assessments at Intake.
Serves as a member of the medical committees, pavilion and program meetings, case evaluation, audit and utilization review committee to provide expertise in the areas of treatment, development, etc.
Attends and participates in all mandatory training.
Performs other duties as assigned or required which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
Requires a valid State of Illinois Physician and Surgeon License.
Requires American Board Certification in the job-related medical specialty of Internal Medicine.
Preferred Qualifications
One (1) year of professional experience evaluating and providing quality medical care to adult patients.
One (1) year of professional experience evaluating, diagnosing and treating adult patients in accordance with best practices.

D for Elizabeth Packard Mental Health Center in Springfield, Illinois to perform highly responsible and exacting professional work as a staff psychiatrist on an assigned hall. Performs and writes psychiatric evaluation and progress notes. Provides support and expertise to staff in working with patients. Provides education in areas of patient health, interrelationship of bio-social issues to patient care, especially in-regards to the use of psycho tropic medications, side effects toxicities, etc. Serves as an active and contributing member on assigned facility committees, staff meetings, teaching conferences and clinical committees. Provides psychiatric services and consultation to other Division of Mental Health hospitals via tele-psychiatry and periodic on-site review of cases. The Department of Human Services is seeking to hire an independent, highly organized, and detail-oriented licensed Registered Nurse for the Illinois School for the Deaf located in Jacksonville, Illinois. This position will be responsible for performing direct nursing care for students with an identified hearing impairment. Manual communication is highly recommended. Essential Functions
Functions as an interdisciplinary team member while providing the highest standard of care to students.
Serves as designated lead worker to lower-level staff.
Utilizes the core elements of the nursing process while assessing students with acute and chronic conditions.
Completes mandatory training requirements and meets Registered Nursing continuing education hours to maintain licensure.

The Department of Human Services is seeking to hire an independent, highly organized, and detail-oriented licensed Registered Nurse for the Illinois School for the Deaf located in Jacksonville, Illinois. This position will be responsible for performing direct nursing care for students with an identified hearing impairment. Manual communication is highly recommended. Position Overview
The Division of Mental Health is seeking to hire a Physical Specialist, Option B for the Elizabeth Packard Mental Health Center located in Springfield, Illinois to serve as primary care physician on a secure forensic unit. Provides medical care and treatment to patients and employees of the facility. Provides consultation to the psychiatrists. Participates actively on medical staff committees.
Essential Functions
Serves as primary care physician for the Elizabeth Packard Mental Health Center on a secure forensic unit.
Provides consultation to the psychiatrists.
Participates actively on medical staff committees.
Provides evaluation and treatment for injured employees.
Participates in all Department of Human Services and Elizabeth Packard Mental Health Center mandatory training.
