Job Description

Cummins Behavioral Health Systems, Inc. is seeking a clinical professional to fill the role of State Operated Facilities (SOF) Liaison in Avon, Indiana.

Job Summary: 

Under the supervision of the Medical Director, the State Operated Facilities (SOF) Liaison will assist the Cummins staff in providing psychiatric services in State Operated Facilities.

The position of SOF Liaison also has components of a Life Skills Specialist who primarily provides face-to-face contact for the purpose of coaching, teaching, or assisting consumers in meeting Person-Centered Treatment Plan goals to improve functioning in major life domain areas. The Life Skills Specialist is an active member of the consumer's treatment team, providing service delivery monitoring, staffing the consumer with the psychiatrist, therapist, clinical supervisor, or other treatment team members, and coordinating care. The services that the Life Skills Specialist provides are under the supervision of a Licensed Professional or a QBHP and are driven by the treatment plan of the consumer. Life Skills Specialists follow best practice treatment models within a Recovery-based perspective that is intended to promote resiliency, optimal community participation, and eventual independence from ongoing professional behavioral health support. Life Skills Specialists utilize the following models and approaches to meeting the consumers' needs:

  • The Cummins Behavioral Health Systems, Inc. Meeting Facilitation Guidelines
  • The Cummins Behavioral Health Systems, Inc. Leadership Principles
  • Person-Centered Treatment Planning (PCTP)
  • Motivational Interviewing (MI)
  • The Wellness and Recovery Model and Approach
  • The Cummins Behavioral Health Systems, Inc. Life Skills Specialist Service Delivery Model
  • Coaching approach to service delivery
  • Other modalities

Essential Functions may include but are not limited to:

Other duties may be assigned. Regular and reliable attendance is an essential function of the position.

  1. Assists Cummins staff throughout the organization in reviewing the necessity for SOF placement and, if appropriate, the coordination of State Operated Facility referrals, admissions and discharges.
  2. Acts as liaison between Cummins, the State Operated Facilities and the families of individuals that are receiving services at a SOF.
  3. Complete all documentation required by the SOF and DMHA to maintain services for individuals receiving treatment at a State Operated Facility.
  4. Responds to State Operated Facilities in meeting time frames and attending meetings to facilitate the services indicated on an individual's treatment plan.             
  5. Meets regularly with individuals at the SOF and the SOF staff  to evaluate the psychiatric status of a given individual, to ascertain their progress toward treatment goals and for discharge planning .
  6. Assists in obtaining medications for those served without funds that qualify for pharmaceutical company medication assistance programs.

Education and/or Experience:

  • Bachelor's degree in human services or related field required.
  • Three to five years clinical experience with at least two years experience in a psychiatric-behavioral health setting preferred.
