POSITION SUMMARY:
Under the direction of the Program Manager of Care Coordination Services, coordinate program strategies to engage HIV+ MSM to increase the self-efficacy to navigate the healthcare system to ensure access to timely and appropriate care and improve health outcomes. The Program Coordinator further provides non-medical case management to the West Hollywood Community member.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Develop, coordinate, and implement outreach activities.
Coordinate data collection and reporting used for monthly reports.
Identify community leaders and key informants to serve as social network contacts.
Complete monthly reports as required by the Options contract.
Implement the core concepts and key components of the Options intervention.
Manage program supplies and purchase supplies as needed for distribution to clients.
Develop level curricula and facilitate group-level workshops.
Chart client progress and reporting through data entry and maintain client files according to program protocols.
Follow up with providers and clients to assess whether a referral was successful.
Assure client confidentiality as defined by HIPPA and APLA Health policy and procedures.
Participate in program quality management processes
Attend community meetings to promote APLA Health programs and encourage client referrals.
Participate in events that promote APLA Health’s services.
Attend regularly scheduled in-service to increase knowledge of resources that facilitate appropriate and relevant referrals.
Provide non-medical case management to West Hollywood Community Members.
Complete quarterly reports for the West Hollywood program.

What you will do!
The Housing Development Coordinator is responsible for leading housing development efforts to address homelessness, create new housing, and preserve existing residential resources for persons with serious mental illness and addiction disorders. This position manages the various contractual arrangements to implement resultant projects. Position scope includes (1) the coordination and ongoing oversight of HSD housing development initiatives for people with mental health and addiction disorders; (2) serving as program contact for housing development awards; (3) representing HSD and providing leadership on behalf of the department on supportive and affordable housing for OHA clients.
Work involves a significant amount of negotiation, coordination, and technical assistance activity with a variety of federal, state, county and local partners, their constituencies and community-based mental health treatment programs in order to accomplish the development of residential programs. This position develops strategies to access funding from a variety of sources to finance housing projects and related operating subsidies and identifies new ways to respond to the housing needs of target populations. This position is also responsible for assisting housing development activities to implement ORS 426.502 through 426.508 pertaining to housing for people with mental illness at the former Dammasch State Hospital campus and the Community Mental Health Housing Fund established with the proceeds from the property sale. Development and dissemination of informational materials both within HSD and throughout the state is required.

What you will do!
The Housing Development Coordinator is responsible for leading housing development efforts to address homelessness, create new housing, and preserve existing residential resources for persons with serious mental illness and addiction disorders. This position manages the various contractual arrangements to implement resultant projects. Position scope includes (1) the coordination and ongoing oversight of HSD housing development initiatives for people with mental health and addiction disorders; (2) serving as program contact for housing development awards; (3) representing HSD and providing leadership on behalf of the department on supportive and affordable housing for OHA clients.

Many of the individuals seeking services at the Highlandtown office have experienced multiple traumas. In addition to intimate partner/ sexual violence (IPSV), many have also experienced violence during the immigration process, violence in their home country, and community based violence that includes discrimination and hate crimes. These complex traumatic events shape how survivors interact with the people around them, and the Administrative Coordinator must be comfortable engaging traumatized people, who may present for assistance in a variety of emotional states and not always be clear about what they are seeking from us. The Administrative Coordinator engages visitors with sensitivity, with safety in mind, and assists visitors in getting to the right person to address their needs in an efficient and confidential manner. The Administrative Coordinator is also primarily responsible for the ensuring the day to day operations of the Highlandtown office site run smoothly, coordinating schedules for coverage, site and community events; managing the use of spaces on site; interacting with vendors; and managing the food pantry logistics. As the third-party administrator of the Flexible Housing Pool, the Center for Housing and Health (CHH) is charged with maintaining a portfolio of quality, readily accessible housing for program participants. The Flexible Housing Pool is a multisector investment in housing that aims to expand the number of units available to people in Chicago and Cook County experiencing homelessness.
The Youth Program Manager will coordinate the Flexible Housing Pool for homeless youth, ages 18-24 who are frequent users of health services, homeless shelters and legal/justice systems. This position will serve as the Center’s liaison to sub-contracted youth partner agencies providing permanent supportive housing and intensive case management. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Program Management & Operations
Collaborate and coordinate with other FHP Managers to ensure a smooth transition from outreach to housing placements for youths enrolled into the program
